Your Privacy and HIPAA
 Standards for privacy of health information were set nationally in the Health Insurance Portability and Accountability act of 1996, known as HIPAA. The purpose of HIPAA is to assure that your health information is properly protected while allowing the flow of health information needed to provide high quality health care to you and to protect public health.
Your Privacy is Important to Us
Our health and patient information privacy policy protects your health information as well as your personal information and adheres to the standards set forth by HIPAA. We will not share your health or personal information with anyone other than you or your legal guardians except as required by law or by your authorization.
In accordance with our Practice Privacy Policy, and by law, we support you in your right to:
- Ask to see and get a copy of your dental health records.

- Have corrections added to your health information.

- Receive a notice that tells you how your health information may be used and shared.

- Get a report as to when or why your health information was shared for certain purposes.

- File a complaint if you feel your rights are being denied or your health information isn’t being protected.

- Decide whether to give your permission before your information can be used or shared for certain purposes such as with your employer.

We may use your contact information within our office to notify you of practice or dental information by mail, phone or electronic correspondence, but we do not and will not share this information with any third party. The only exception to this would be a service provider who would be used by us to deliver our communication to you, such as a mailing or billing company.
